Whistleblower Window

Whistleblower window is a communication tool between CMA and the reporting person who reported any information or violation or illegal activity or unethical act which would cause damage to shareholders or the company or the public in general. It is preferred the reporting person attach all available documents supporting the report.

Terms and conditions

  1. The complaint should be related to an entity under control by CMA.
  2. The reported person should agree to provide any additional information that may CMA would request to process the investigation.
  3. CMA will not investigate the complaints against entity other than under its control.
  4. CMA is not required to investigate the anonymous complaint or reports.
  5. CMA is not required to update the reported person with process of investigation.
  6. The reported person should keep all the information introduced confidential.
  7. CMA will deal with the information provided in strictly confidential way.
  8. CMA can dismiss the complaint or report in the following cases:
    • CMA investigated it already.
    • Not include actual facts and evidences
    • The cases that seems immaterial.
    • For any other reasons.

Required Documents

This database will provide an access to the data of valuation companies, included the following:

  1. General information, i.e. (location, license data, legal form details etc.)
  2.  Branches.
  3. Employees.
  4. Founders.
  5. Documents. i.e. (CMA license, CR papers... etc.)
  6. General documents. i.e. (rules of valuation of canceled vehicles due to accidents).

Required Documents

 

-          Fill the assistance application form prescribed by the Committee and submit to the Institutional Services Department.

 

-          Provide all the statements and documents stated in the Fund’s procedures.

 

-          Copy of ROP report on the accident showing time and venue of accident, how it occurred, identification of the person who caused it, names of the drivers, address of the owner of the vehicle that caused the accident (if any)

 

-          Copy of identification of applicant (ID/driving license/residence card) copy of Mulkiyah of the damaged vehicle and the vehicle that caused the accident if known, copy of IDs of all parties if known.

 

-          Death certificate of the deceased due to the accident.

 

-          For bodily injuries, attach medical report issued by Ministry of Health licensed hospital showing description of the bodily injury.

 

-          Treatment invoices issued by approved hospital for compensation of treatment expenses. If treatment is abroad, provide evidence from the Ministry of health that treatment in Oman is not available.

 

-          For financial damage, attach copy of technical report from ROP specifying the damage to the vehicle.

 

-          Copy of power of attorney for the heirs

 

-          If the vehicle that caused the accident is unknown, provide evidence from ROP that it is unknown for more than three months from the date of the accident.

 

-          Any information, statements or documents the Committee requests.

 

-          The application shall be one of the cased the fund has the right to intervene.

 

-          The application must be submitted before two years lapsing from the date of the accident.
